Assessment Visit
We start with a comprehensive home visit to get to know you and understand what matters most. This helps us build a professional and caring relationship from day one.
During the assessment, we look at your needs, lifestyle and preferences so we can shape truly person‑centred support. We welcome a next of kin, relative, social worker or health professional to join so we gather the full picture.
We also review any moving and positioning equipment and the home environment to make sure every aspect of care delivery is safe for you and the care team.
Care Planning
The information gathered during your assessment is used to create a comprehensive, person‑centred care plan.
Your plan reflects your life story and gives our team a clear record of your needs and preferences, so care is delivered exactly the way you like.
Once drafted, we review the plan with you at home to make sure it feels right. If anything changes, we’re just a phone call away and will update the plan so it always reflects your latest needs and wishes.
Care Review
We regularly review your care, typically every three months or sooner if needed. This helps us stay close to your day‑to‑day needs and make timely adjustments.
Reviews are a chance to talk through observations or any changes you would like. A loved one or health professional can be present if you wish.
